The Adult Community Nursing Team work autonomously to provide specialist nursing care to adults needing support due to a long term chronic condition or an acute episode of ill health.

About our service

Our Adult Community Nursing team provides 24-hour community nursing to:

  • Patients registered with a Wakefield GP, either in the patient’s home, in a clinic setting or care home
  • Temporary residents within the area if they have registered with a Wakefield GP
  • Transient traveller populations who may require care during their stay.
     

The service provides individual, personalised care to the patient, involving their family, carers and referring to other healthcare teams if necessary.


The Community Nursing team also works with other health and social care staff to:

  • Provide clinical triage to ensure the right advice is given and the appropriate professional responds to the patients need.
  • Provide safe and effective care using a multi professional approach
  • Reduce unnecessary hospital admissions, support quick and smooth discharge from hospital and keep people in their own homes as long as possible
  • Work with other professionals, patients and their carers to provide consistent care
  • Work with patients to be as independent as possible and help them manage their own care
  • Provide advice and recommend additional healthcare services to patients, family and carers.

Conditions we treat

We provide a wide range of services for patients including:

  • The diagnosis and treatment of skin cancer
  • The treatment of general dermatological diseases such as acne, eczema and psoriasis which your GP is finding difficult to manage

Allergy testing:

  • Patch testing
  • Consultant-led hair clinics
  • Paediatric eczema nurse led clinics
  • Minor surgery
