Wakefield Intermediate Care Unit

What we do

We provide seven day short term in-patient nursing and therapy for patients in the Wakefield District who are in hospital and need extra support, care and rehabilitation before they go home or to the place where they normally live.  Referrals to the unit are from hospital therapy and nursing staff.

Who we are

Nurses, health care support workers, doctors, occupational therapists  and physiotherapists carry out assessments and regular reviews with the support of social workers, dieticians and pharmacists to ensure individuals are safe to be discharged to home or their preferred place.  We also have links to;

Age UK
Carers Wakefield
District Nurses

The unit, which is still undergoing refurbishment,  is on two levels. All the rooms are single rooms with a sink and TV. Commodes can be supplied if needed. The six rooms on the ground floor are for patients requiring more intense nursing and therapy input.
