Our community teams take great pride in providing delivering patient-centred care, predominantly in patients’ own homes in the Wakefield and district footprint.

Our adult community teams of highly skilled district nurses, senior nurses, staff nurses and health care assistants care for patients at various stages of their healthcare journey, helping them to manage their illness at home. Our teams are trained to deliver a range of clinical treatments/interventions such as wound care management, end-of-life care as well as working with GPs to deliver ongoing management of patients with long-term health conditions. Self-management and promoting independence is at the heart of everything we do, and a large part of the community nursing role is to work closely with GPs to manage and support patients with complex medication requirements. 

Working in partnership with our other adult community services, our integrated care team works 24/7, 365 days per year providing nursing/care visits to our patients, supporting early discharge from hospital and preventing unnecessary hospital admissions through providing support in a patient’s own home. We also have an urgent response team, which responds to urgent nursing and therapy needs, as well as supporting patients on discharge from hospital. Referrals can be received from acute hospitals, general practitioners, patients and carers. 

In addition, we have the community-based MY Therapy rehabilitation service, consisting of physiotherapists, occupational therapists, dietitians and therapy support staff, whose aim is to facilitate hospital discharge and support patients to optimise their independence in their place of residence. Its speciality services include: a neurology team, a specialist respiratory physiotherapy service, a specialist cardiac rehabilitation service, generic therapy services and a specialist dietitian service.

We also have the Wakefield Intermediate Care Unit, a 26-bedded inpatient rehabilitation unit, which provides nursing care 24/7 accompanied by therapy input for those who need extra support, care and rehabilitation before they go home or to the place where they normally live.

Our community paediatrics team consists of therapists, paediatricians, nurses and specialist nurses, who work collaboratively with our hospital-based acute paediatric teams and other specialist community clinicians and services to provide expert care. The team has experience in many areas, including children with autism spectrum disorders, complex learning difficulties and behavioural or developmental issues, as well as providing medical expertise for things such as adoption processes, youth offending, continence and nurse services for sleep disorders. The paediatric service also has excellent links with the schools and special schools in the area.
