
The Wakefield Intermediate Care Unit multi-disciplinary team provides short-term bed-based support to patients to help them become as independent as possible to return home after a hospital stay, or alternatively to support them to choose the next steps for their care.
We provide short term in-patient nursing and therapy to patients in the Wakefield District who need extra support, care and rehabilitation following an acute hospital stay before they go home or to the place where they normally live. Referrals to the unit are from hospital therapy and nursing staff.
Nurses, health care support workers, doctors, occupational therapists and physiotherapists carry out assessments and regular reviews with the support of social workers, dieticians and pharmacists to ensure individuals are safe to be discharged home or to their preferred place.
The unit is on two levels. All patient rooms are single rooms with a wash basin and TV. Six patient rooms are on the ground floor and are for patients requiring more intense nursing and therapy input.

Mobile phones, MP3 players and laptops or tablets are permitted; however please note that the Trust will not accept responsibility or liability for patients’ property brought into the Trust’s premises, unless it is handed in for safe custody and a copy of an official patient’s property record is obtained as a receipt.
