Neurology Therapy

Our neurology therapy team consists of physiotherapists, occupational therapists and technical instructors who provide therapy for our patients with a neurological condition across Wakefield. 

About us

Our team support people in their own homes, care homes and within our outpatient clinic settings. Our team aim to support you to achieve your goals and may progress you through from community to our clinic setting as you progress. 

Our team can help with lots of different challenges caused by neurological conditions, for example:

  • Changes in walking
  • Changes to independence with activities around the house
  • Requiring a splint due to changes in hand/wrist positioning
  • Changes to balance or dizziness
  • Advice and support to return to work

The neurology team triage referrals made by GPs, consultants and patients themselves. A coordinator will be in touch to discuss your therapy needs and arrange an initial assessment as appropriate. 

Early Supported Discharge Team (ESD)

If you have been in hospital following an acute stroke, you will be discharged with a referral to our ESD team. The team review referrals Monday to Friday and will make contact with you within 24 hours of receiving the referral and aim to contact you within 72 hours. 

Our ESD team can support patients with their rehabilitation journey for up to 6 weeks following discharge from hospital and can visit 2 times a week to help support in achieving your goals. Our therapists will discuss your goals and help set a therapy programme to help you achieve what is important to you. This may include exercise programmes, cognitive rehabilitation, functional assessments and practice or aids and adaptations. 

Following 6 weeks of input, our therapists will discuss ongoing goals and discuss appropriate onward referrals either to our neurology team for at home or clinic input or support from external agencies such as the Stroke Association.
