Community Integrated Care Team (ICT)

The Integrated Care Team (ICT) is a community based nursing and carers team that works 24/7, 365 days per year providing nursing/care visits to patient that are registered with a Wakefield GP in the district surrounding area, providing support in a patient’s own home.

Our ICT provide:

  • Rehabilitation at home - supporting early discharge from hospital
  • Rapid response - preventing unnecessary hospital admissions
  • End of life care  - for patients wishing to be cared for in their own home
  • Intravenous antibiotics in own home
  • Out of hours district nursing visits

key facts

In 2020/21 the team completed 31,016 face to face visits and accepted 1,591 new referrals onto the service.

The service saved 1,687 bed days in 2020/21 due to administering intravenous antibiotics in 147 patients own homes.

Current project

The initiative involves partnership working between The Integrated Care team (Nurses), the Urgent Response Team (Advanced clinical practitioners), Pharmacy antimicrobial stewardship team and Rachel Holt (Consultant Geriatrician).

Care home residents that are not suitable for hospital admission who have a bacterial infection (possible chest or urine) are referred by their GP to the Urgent Response Team for review if suitable to take part in the pilot.  ACPs review resident, discuss with Elderly Care Consultant and if appropriate will receive 3 days of IV ceftriaxone then step down to oral antibiotics.  Resident referred to ICT by AMS team to cannulate and administer IV antibiotics.
