Urgent Community Response

Our urgent community response team provide urgent care to people at home to avoid unnecessary hospital admissions, aiming to enable people to live independently for longer.

The team consists of Advanced Clinical Practitioners, Occupational Therapists, Social Workers, Continuing Health Care, General Practice Equipment Services and Voluntary Services to ensure people are kept safe at home.

Adults with complex health needs who urgently need care, can receive fast access to a range of health and social professionals within two hours or in the same day.

About our service

Our Urgent Community Response Team visit people in and around Wakefield who have a Wakefield GP and are over the age of 18 years.

The service is covered 24/7 365 days per year via single point of contact 01924 327591

Calls are triaged by health professionals who will ensure the right service meets people’s needs. Needs may be related to:

  • Falls
  • Frailty
  • Reduced function/deconditioning/reduced mobility.
  • End of life/Palliative crisis support.
  • Urgent equipment provision to support a person experiencing a crisis/at risk of hospital admission.
  • Confusion/Delirium.
  • Urgent Catheter care.
  • Urgent support for diabetes
  • Unpaid carer breakdown which, if not resolved, will result in a healthcare crisis for the person they care for
