Single Point of Contact (SPOC)

Single Point of Contact (SPOC) is a point of contact for people with a Wakefield GP using our services. This is a single telephone number but we endeavour to prioritise calls that require an urgent response or people, their family or carers particularly those at the end of their life.

Single Point of Contact (SPOC) in Wakefield, is a point of contact for adults registered with a Wakefield GP who are using our community health services. Or for all referrers, families or carers supporting people using our services.
