Single Point of Contact (SPOC) in Wakefield, is a well-established point of contact for adult (over 18) community health services for patients (and all referrers acting for patients) that are registered with a Wakefield GP.

About us

Single Point of Contact (SPOC) in Wakefield, acts as a point of contact for adult community health services for all referrers acting for patients registered with a Wakefield GP. The service is a single telephone number for both referrals and queries.

The team is able to signpost to the correct service from a range of Mid Yorkshire Hospital Trust’s adult community health services. Priority is given to those calls that require the most urgent response with particular priority given to calls from those patients at the end of their lives as well as their family and carers.
