District Nursing

The District Nursing team provides a 24-hour service, 365 days a year, caring for and supporting patients in the place they call home. 

The teams include district nurses, who are qualified specialist nurses, community staff nurses, nursing associates and clinical support workers. They work together to improve or maintain the health of mainly housebound people who are aged 18 and over who are registered with a Wakefield GP. This involves working in partnership with GPs, social care providers, hospital and hospice teams, and other third-party organisations to ensure that whenever possible, care can take place.

What we do

A referral to the District Nursing team can be made by your GP practice, when you are discharged from hospital or by a social care professional. At your first visit our nurses will carry out an assessment and work with you and your family or carer to discuss goals and agree plan of care, supporting you to remain as independent as possible.
