The Neighbourhood Care Coordinators (NCC) team is part of Mid Yorkshire Teaching NHS Trust within Adult Community Services.

Neighbourhood Care Coordinators (NCC)

We help adults in Wakefield district with social and wellbeing support. 

The NCC is a non-clinical team that acts as a liaison for health, social care and community services and provides a free service which is accessible to all adults in the Wakefield district.

We can:

  • Listen to your needs and offer friendly support. 
  • Meet you in a place that suits you and your family or carers.
  • Talk through your situation and help you find the services and community groups available to you 
  • Connect you with local help for health, home and daily life. 

What we can help you find support for the below services:

  • Personal care (e.g. washing, dressing)
  • Help with housework or cleaning
  • Befriending services to reduce loneliness
  • Money advice and financial support
  • Mental health support
  • Local activities and classes
  • Dementia care
  • End-of-life support

To refer yourself or someone else, call:

Mid Yorkshire Single Point of Contact (SPOC) on 01924 327591 and ask to speak to the Neighbourhood Care Coordinators.
